Here's some tips for when the action actually heats up at the palace:
Soris, your cousin and party member, has a sword and shield spec, so be sure to equip him with them to take advantage of his enemy-crippling Shield Bash. (I learned that bit late and found it made a huge difference.)
Give Soris AI command to heal himself when necessary. He can be pretty vulnerable if not careful.

I also recommend switching to crossbow for long-range enemies instead of charging them, you have 2 to often his one. hehe But that's up to you.
Dogs (Marabis) will actually be tougher than people so concentrate on those first if possible. Though you may want to divvy up the enemies it may be best to gang up on one enemy at a time to whittle them down faster.
I realized too late that I may be able to steal keys from people and guards. So if you're a rogue you may want to try which could unlock the "key required" metal doors.
Every time you see the game auto save for you, prepare for a big battle. The last encounter before Vaughan will be his bodyguard who will rush you from the end of the hallway with an ice spell leading his charge. He will yell so you should be able to dodge him. This will be I think the third big encounter so just stay alert.
Vaughan himself is easy enough. I didn't take his money so don't know if you can avoid a fight. But the fight itself wasn't too hard. Soris will take on Vaughan's lackeys. You should focus on Vaughan as he definitely has a hard on for you. As a rogue, the talent Dirty Fighting will definitely come in handy in stunning him for a few seconds at a time. Below the Belt will also help knock him back a bit.
Once you finish the fight go back to the Alienage and agree to be part of Duncan's Grey Wardens. Leave with Duncan and you've finished your origin. Congratulations! Solution by Wayens on TA

Your party members are pretty strong this time around so on Normal, at least, I mainly kept in mind my own health and positioning, like for a rogue, behind enemies is the best for his backstab move.
With the boss at the top of the tower, it will be harder. I ended up losing my party and barely got in my last stab to finish him off. And this is on Normal difficulty. So for you guys, I recommend setting an AI command to heal when they need it to keep your party in the game.
Recommended talents to have (for rogues at least) are Dirty Fighting (stun), Deadly Strike (heavy blow), and Below the Belt (knockback).
Don't be afraid if you see your back on fire. The mage, I think, enchants your weapons to Flaming for extra boost. Enjoy it. ;)
TIP: Let loose the dogs from their cages and make use of the ballistas to even the odds a bit if needed. Be careful of the grease trap in the "fire chamber" (you'll see what I mean).
NOTE: Any difficulty above Normal leaves you vulnerable to friendly fire. The mage has area spells that can be detrimental if you get caught. Be careful.

Humans
The humans are the only one that as far as I know you don't have a choice about as I believe you have to save the Arl of Redcliffe. This achievment will unlock once you go to "landsmeet". Hope this helps.

Rabble-Rouser 20 Completed "The Landsmeet"
This is story related and can not be missed.
After you have completed the quests to gain support of the Elves, Dwarfs, Mages and the Arl of Redcliffe ask the Arl to convene The Landesmeet.
This will start up the major quests in Denerim. Once you have completed these travel to the Royal Place and confront Loghain. Once this is done the achievement is yours.
One note if you plan to reload saves in order to get all the different endings in one playthrough your best bet is to save before travelling to the Palace. This save will also be useful if you find you have missed anything for the Traveller achievement as all the missable area's and buildings can still be reached before the Landsmeet.

1stly morrigan is easy i think you just get her high enough with gifts if not go to broken circle quest and get the grimoire? gift it her that will start her "personal quest" which i wont spoil but pretty much after that she willl be.... preceptive.
Zevran is a whore flirt and hes yours.
Alistair is a virgin and needs to be treated nicely, he will eventually gift you a rose then talk about his sister his "personal quest"
Lastly leliana she needs be 100% i think in order to share a night of passion, like the others talk to her alot shes mention her old mentor/lover untill while you'll travelling you will be atacked by assassins from orlais starting her personal quest
Complete that and back at camp she talk about her old mentor then a line will come up "shes was special to you, i hope one day i can be as special has she was to you" or similar that nails you the romance and cheevo.
I usually find you get to sleep with party members after 2 stories i.e get 2 factions join your army or 1 and redcliffe 1st part.

There is 4 possible endings that you need to view so you can unlock this achievement, they are as follows:
1. Towards the final moments of the game Morrigan will confront you after you learn how to defeat the Archdemon. She will offer you a solution to killing the Archdemon without having to sacrifice any Grey Wardens. if you take her up on this offer you will unlock a 50g achievement and will have discovered another ending.
2. Without taking up Morrigans offer, defeat the Archdemon and sacrifice the main character.

3. Without taking up Morrigans offer, defeat the Archdemon and sacrifice Alistair.
 4. During the landsmeet a fight will break out between the main character and thier party and Loghain. When the fight is finished an offer between Loghain and a character of your choice to dual is presented. When you are choosing who to use at this point select your main character. After the fight is over you will have the option to either execute Loghain or to make him a Grey Warden. Choose to make him a Grey Warden (Choosing to make him a Grey Warden Removes Alistair from your party). Now without taking up Morrigans offer, defeat the Archdemon and sacrifice Loghain.

Numbers 2, 3, or 4 will also earn you another 50g achievement

If your character starts out with a tome in their inventory (or if you have one via DLC) select it in your inventory (under usable items) and press A, then exit your inventory. Upon returning to the game the achievement will pop. Alternately, open your menu wheel, select potions and go to the end and tomes are there.
I only know for certain that the Human Noble starts with a tome, a book of family history

Magic Sympathizer 20

You can get this and "Annulment Invoker" with a minimal amount of replay (just one or two dialogues) if you delay the moment of decision as long as possible.
When you enter the mage's circle tower and meet Wynne, side with her and get her to join your party. Then play through until you free First Enchanter Irving. He will give you the choice to proceed back down to the Templars or wait, choose wait and then save your game.
To get "Magic Sympathizer," when you talk to Greagor, the Templar leader, tell him you trust the Circle is back in order. Then talk to Irving and after your conversation this achievement will unlock. If you haven't gotten "Annulment Invoker" by this point, re-load the game from your save and follow the instructions I left for that achievement. This part by ianthe on TA
 
I read the previous soultion but i did it even easier, just before you fight the last "boss" at the top of the tower save.
During the fight if you dont save any mages at all you get templars + annualment invoker cheevo, if you save a few stand by a glowing mage and bring pause menu up and choose the tablet you get mages!

If you haven't gotten "Magic Sympathizer" by this point, re-load the game from your save and follow the instructions I left for that achievement.

Here's how you can get "Harrowmont's Ally" and "Bhelen's Ally" in one playthrough.



Note: I'm paranoid about closing storylines off too early, so I usually delay picking a side until the very last minute. So there might be a quicker way to do this, but this method definitely gives you a decision point where you'll only have to replay about two minutes of story in order to get both achievements.


First of all, the setup: I basically played both sides as far as gaining their trust. I accepted the initial tasks from both houses, got both of Harrowmont's champions to fight again, and turned both of Harrowmont's allies into enemies for Bhelen. At that point Harrowmont's emissary got mad but I agreed to play double agent for him, and cleaned out dust town.


Now you should be in a position where you're still begrudgingly trusted by both factions, and they'll both try to get you to go find Branka in order to get her endorsement. You'll also be finding out that Bhelen's pretty much a tool, but that's beside the point.


Play through the entire Anvil of the Void storyline. Whether or not you choose Caradin or Branka, you'll be given the opportunity to have a crown made so that you can basically choose the next king. After you get the crown, SAVE the game. This is your decision point. Leaving the anvil room you'll be given the choice to fast travel directly to the council chamber, where a cutscene immediately plays.


For "Bhelen's Ally" declare that Branka or Caradin chose Bhelen to be the next king. Harrowmont will concede, and Bhelen will prove how much of a bastard he is by having him executed. After you wash the bad taste out of your mouth at having basically given this guy the keys to the kingdom, he'll promise to help you with troops, and the achievement will unlock.


If you haven't gotten "Harrowmont's Ally" at this point, just reload from your save and follow the instructions in that achievement guide

Liberator 20

When you get to the "Anvil of the Void" area in the Deep Roads, you'll eventually come to a room with four anvils surrounding a column with four faces. After you defeat the trial in that room, SAVE your game, because you're at your decision point. Through a tunnel and into the next room a cutscene will play and you'll have to choose between Caradin and Branka.

For "Liberator" you need to side with Caradin rather than Branka. Keep on picking the choices that lead to destroying the anvil. After the cutscene Branka will go postal and you'll have to fight a pretty messy battle, followed by another cutscene, and the achievement pops.


At this point if you haven't gotten "Pragmatist," load from your save and perform the steps in that achievement guide.

 This one can be a bit confusing depending on what you read on the internet. However it is fairly simple. You need to set foot in every area of the game which means visit every area. Where people miss this is in the side quests, some areas can only be visited by doing side quests. For simplicities sake I will only make reference to places where you don't have to go to complete the game.
Edit: I have played multiple playthroughs of the game and to my knowledge you have to do this in one playthrough.
2nd Edit: just to clarify, the percentage of the world you have explored doesn't mean anything in relation to this achievement, I had one playthrough at 96% explored and no achievement and I popped it when I had 94% explored. Just as a recommendation I would also recommend visiting every house, etc. in Denerim and Orzammar. Some people are also reporting you don't have to do all these sidequests, but this is a list of all of the known sidequests, it obviously doesn't hurt to do too much rather than too little ;)
Note: Spoilers ahead!
Sidequests:
Civil War: Found by accepting a mission on the chantry board in Denerim
Battlefield: Same as Civil War
Dirty Back Alley, Rundown Back Streets, & Dark Alley (Denerim Sub-areas): Take the mission "Back Alley Justice" from the Denerim Chantry board.
Deserted Building (Denerim Sub-area): Find a note on Ser Friden's corpse, it will unlock a new mission which lets you go to this area.
K's Hideout: Subquest by talking to the bartender in the Market District, ask him for activities that aren't legal, perform all the subquests and you will eventually unlock this sub-area in Denerim.
Refugees: Accept a mission on the chantry board in Redcliffe
Caravan: Same as Refugees
Kadan-fe Mercenaries: Assassination subquest, you must recruit Zevran and talk to a merchant in the hub in the Denerim Market area, a boy will give you a message on your way out of town. Do the assassination quests for the crows and you will unlock this area in the main map. (This area disappears after you complete the mission)
Ransom Drop: Another "assassination" quest, this is a sub-area in Denerim
Areas that can be missed while playing the game:
Aeducan Thaig: Area in the deep roads, you don't have to visit it for the storyline, but you do have to visit this area. This area is always available
Fort Drakon: You HAVE to let yourself get killed while rescuing the Queen so you can be arrested and transported here. Edit: I've also been told there is a way to let yourself be arrested, either way you have to be arrested whether you try to fight your way out after rescuing the queen or let yourself be arrested.
The Fade: You have to either sacrifice the mother or get the circle of magi's help in "The Arl of Redcliffe" mission so you can be transported here
The Fade pt 2: After the sloth demon puts you to sleep in the Magi Tower, you have to open up 5 areas to get to him to fight him, however you will unlock 3 outer areas you do not have to visit to fight him, but you will get your companions to help you fight the demon - visit all 3.
The Final Battle: After the Landsmeet and are fighting the final battle in Denerim, make sure you visit the areas in this order: Market District, Elven Alienage, Royal Palace then Fort Drakon.
This achievement will unlock when you go on to the roof to fight the archdemon (took about 20 seconds for me after I was on the roof). You do NOT have to have the DLC to get this achievement. Hope it helps

These three achievements, Grey Warden, Master Warden, and Blight-Queller, are gotten from killing Darkspawn, respectively 100, 500 and 1000. This can theoretically be done in one playthrough, but require some luck and finding random encounters when travelling.
Luckily, the key here is that the achievement description should have an added "Across all playthroughs", as the count will tally up how many you have killed on all your characters. Saving, killing darkspawn, and then reloading to kill the same again will not count.
To see how far along you are, with the back button, find the leftmost book icon and then scroll right to Heroic Accomplishments. On the Party Accomplishments page, look at kills and Darkspawn killed, on all your characters, for the grand total.
If you are close to 1000 but have finnished the game, the easiest would probably be to start a new Dwarf Noble story for some quick access to more Darkspawn, or play through to Ostagar and then do the Korcari Wilds and Tower.

Here is a solution by Shinerbock88 on TATips for Nightmare Difficulty:

This really isn't that difficult if you have your AI set up correctly and/or you use the radial menu, you have a healer, a hex mage and a good tank. I used Leliana with ranged attack as my fourth character. Your healer doesn't even have to be a Spirit Healer, though Group Heal is nice to use right after a regular Heal if your tank gets low on health. If you just have Regenerate and Heal, you're golden. My characters were Lv13-14 and I had 3-5 levels of level up points to distribute, so it's really not that bad of a fight... I don't use the Level Up function until I have to so I don't get caught in a part of the game I cannot get through with certain abilities. Plus I was waiting on getting my second specialization for every member and I wanted to flood those specializations immediately. I don't even have any big-damage ability chains to help out here. If you do, by all means, use them.
First, set up the battlefield before you hit the Gong. Put your entire party on HOLD and position them individually. Set up all your sustained spells and get ready. I'm going to list my set up to give you sort of an idea how I did it. It's likely our party make up is not identical, so bear in mind you'll likely need to make some adjustments. The tank is the main focus of your positioning because it's crucial to keep High Dragon's abilities away from your other party members and make it so only the tank gets hit.
Tank:

As you look towards where High Dragon lands, place the tank to the left side of where High Dragon lands and get ready to Taunt. High Dragon can toss your tank around like a wet noodle, so putting him here keeps the cliff at your back and High Dragon can't throw you around and away from your party members or out of your healer's range. If your tank moves anywhere, it will be towards the Ruin Entrance which is fine because your other party members can still target High Dragon and the healer can heal the tank from the ledge above.
Threaten helps out a lot with threat as well, so activate it from the sustained spells menu on your radial menu if you have it. You should have this if you have a tank and if you have Threaten, then you have Precise Striking which will help with damage and threat as well because your tank hits more often and has a better chance to land crits: Less misses + more crits = more threat. Again, activate it if you have it.
Set up the tanks AI to use Shield Defense if their health drops below 50%. The reason to wait is because Shield Defense nerfs Attack and we don't want that unless we have to. Every once in a while, you should swap to your tank and Taunt again.

Use health items to help your healer out when you get lowe on health.

Also, any further into the ability string of the abilities I mentioned above will just make your fight easier. I am just trying to get you through with the bare minimum.

The rest of the party:

Your other three party members (if ranged) need to stay on the ledge where the Gong is to keep them out of the High Dragons AoE and cone abilities range. They can spam spells and ranged attacks from this ledge.

If one is melee and not ranged, try to keep them attacking from the side of High Dragon because it has a Tail Swipe ability (dunno the actual name) if you're behind it and it will knock you down and a good distance away from the fight. I never got hit with it from the side in 5 fights, but it could happen I suppose.

Hex Mage:

The hex spells from the Entropy school helps a lot here. Be sure to save your Misdirection Hex for when your tank ends up in the mouth of High Dragon or if your healer is low on mana and needs to drink a Lyrium Potion (or the mana Blood Magic spell you get from DLC). This will help alleviate some of the damage.

Alternate your Affliction Hex and Vulnerability Hex (this helps Drain as well if you find your self needing to heal after using the mana Blood Magic spell). Once you land one of your Hexes, spam your elemental spells (Lightning, Fire, Winter's Grasp, etc.). Those hexes lower the resistances of such spells and causes them to do more damage. It helps elemental based weapon enchantments and spells like Frost Weapon and Burning Weapon as well.
Healer Mage:
Not much to say here, keep Regenerate on your tank at all times. Don't wait for health to get low before using Heal. Use offensive spells to help with damage when you can, ESPECIALLY after a hex. If you're a Spirit Healer, then don't be afraid to use Group Heal immediately after Heal even if your other party members have full health (they should take very little damage if any).
Ranged/Melee member:
Leliana with the archery abilities would help a lot here. Ranger Spec, even better. I went the rogue route with her, so my ranged abilities are still default and they offer nothing to this fight. She is a decent archer with Marjolianne's bow (I know that spelled wrong, sorry). If you have elemental arrows (e.g., Ice Arrows), use them and your hexes will help those too.
Ending Comments:
This is a fairly long fight if you have to whittle away at High Dragon's health little by little like I did. Expect at least a 7-10 minute fight. Don't give up! If one thing doesn't work, try something else. Make sure your characters stay put by using using the HOLD function. Keep all threat on your tan, keep your tank healed and *DING!* Dragonslayer.

Defender 20 Preserved the lives of half the troops at Denerim's Gates in "The Final Battle"
After picking the 3 characters you're going to take to the final boss fight with the Archdemon during the Denerim climax, you'll be faced with a number of battles through the Market District, the Alienage and the Royal Palace before arriving at Fort Drakon.
During these battles you have to ability to summon armies within the 'Advanced' section of the Radial Menu. To unlock this achievement, you must keep half of these armies alive and it unlocks upon arriving at Fort Drakon if I remember correctly. During the Archdemon fight you can use the armies as much as you want.
The most certain way to get this is to just not call out any of the armies. It's not even necessary on the easier difficulties anyway.

When playing the Warden's Peak DLC, near the end of the tower portion, there is a potion sitting on a table with a book. If you interact with the potion, you get three options. Drink it, destroy it, or leave it alone. Drinking it will unlock the achievement, as well as give your character a couple of new skills

Golem's Memories is a sidequest from Stone Prisoner DLC, I unlocked the sidequest by taking to Shale after "A Paragon of Her Kind" Where the paragon Cainan reconises Shale and tells him about his past. I don't know how this unlocks if you dont have Shale as a party member when you meet Cainan.
